Surgical Treatment for Portal Hypertension with Variceal Bleeding in Cirrhotic Patient

Abstract
A few author reported a study on portal hypertension and they pointed out some differences between
Korean and American population.
The present study was a clinical analysis of 33 cases of portal hyportension with variceal bleeding in
cirrhotic patients:
The following results were obtained.
1) Of thirty three cases, twenty six patients underwent elective surgery and the remainde had emer
gency operation. Splenorenal shunt was the great majority of surgical procedure and were performed
upon twenty five patients.
2) The preoperative mean pressure of portal vein was 450§®H©üO and decreased to 300§®H©üO
following splenorenal shunt.
3) The overall mortality rate was twenty five percent and 71 percent in emergency surgery and
12 percent in elective procedure. Partuculary the old age and liver function impairment influenced upon
mortality rate. The death were rebleeding (2 cases), hepatic failure (3 cases), acute renal failure(2
cases) and pneumonia(1 case).
4) The postoperative complications were fever(10 cases), infection(8 case), ascites(6 cases), rebleeding
(3 cases) and encephalopathy(2 cases).
5) Splenorenal shunt appeared to be procedure of acceptance in treatment of portal hypertension with
cariceal bleeding in Korea cirrhotic patient.
